Perforation Repair.

Lots of interesting cases this week.

This tooth had the endo done at least 10 years ago with the post and crown soon after. The pt had no trouble until this year when the tooth flared up and required antibiotics. The preop shot shows the post perforating the distal root.

The post op film shows the tooth after crown access, post removal, perforation repair with MTA and retreatment.

I’ll show the recall in 1 year.

    nice furcation involvement.i’m looking foward to seeing the recall films!

    btw, what instrument do you use to take off crowns and bridges in one piece? reverse mallet, or one of those fancy gizmos that are out now? i know there are several types but i’m not too sure of the names – you make an access hole thru the top of the crown and use some sort of suction or screwing motion to break the seal and life the crown off the tooth. or do you just access through the crown?

    I’m optimistic about the furcation involvement because of the lack of preop probing.

    I have a pneumatic reverse hammer in the office that I rarely use. I know about the other gadgets but I usually access through the crown or bridge. This is usually only a problem for procelain on the lower anteriors because of how skinny these teeth are. I find accessing safer for the restoration and for the tooth.

    Any removal instrument that requires drilling through the crown is pointless if I can do my stuff through the crown anyway.
